The Citrus Heights Mission

The Citrus Heights Post Acute mission is to provide long and short-term care, skilled nursing, rehabilitation, and respite care in a safe and welcoming space. We offer a talented staff, state-of-the-art equipment, and nursing practices that adhere to the highest standard of care. You’ll also find amenities like a large number of private bedrooms, bathrooms with showers, expansive common areas, and a wide variety of activities to support the social and emotional needs of our residents.

Our facility features more private room options than many other providers in the area, allowing residents to enjoy greater comfort, privacy, and peace of mind throughout their stay. We also offer an expanded parking area, making visits from family and friends more convenient and accessible.

The Citrus Heights Post Acute care team includes a variety of skilled, dedicated professionals working together to help patients get back to the highest level of function.

Nursing/Clinical Staff

Led by the Director of Nursing, our nursing staff delivers care and clinical services according to quality standards. Registered Nurses (RNs) assess, plan, provide, and document nursing care. They oversee Certified Nursing Assistants (CNAs), who assist patients with activities of daily living including administering medications, eating, bathing, dressing, and transferring.

Therapy / Rehabilitation

Unlike facilities that rely on contracted therapy providers, Citrus Heights Post Acute maintains a full on-site rehabilitation team. Because our therapists are employed directly by the facility, residents work with the same therapy professionals throughout their stay, providing continuity of care from admission through discharge.

Social Services & Activities

The Social Services team leads and plans programs that address the mental, social, and emotional needs of patients and families. Social Workers identify and coordinate individual services to maximize independence, dignity, and quality of life. The Activity Director leads activities for residents.

Dietary Services

Registered Dietitians (RDs) oversee meal planning and presentation to ensure nutritious, tasty meals that fit each patient’s needs.

The Environmental Services team oversees facility maintenance, laundry, general housekeeping, and cleanliness according to health and safety standards.
