We would never recommend Citrus Heights Post Acute to anyone. A family member had a stroke and was moved from the hospital to CHPA, where he spent five weeks. This is where he passed away. We had high hopes for this facility, but it was a disturbing, frustrating experience dealing with the staff. Once hospice got involved, they seemed to know nothing about working as a team with hospice. On our daily visits, we often found our family member very agitated and in need of meds, unclothed, in a soiled adult diaper, or bleeding from an unattended wound. Politely asking for assistance was met with apathy. CNAs and RNs acted as if we were bothering them. One day when our loved one had managed to tear off his adult diaper, the RN at the counter told me, “Well his nurse is at lunch and will be back in 25 minutes.” Unbelievable! The correct response should have been, “His nurse is at lunch, BUT LET ME FIND SOMEONE ELSE TO HELP,” and then to actually follow through. It’s ironic that this is a CARE facility, but the staff doesn’t seem to CARE about their patients. Hey, we get that this type of work is not easy by any stretch, but we were hopeful that staff would do their jobs and see to our loved one’s care. (Our one shoutout: Maritza was a sweet, lovely CNA who actually seemed to be on staff for the right reasons — to care for people.)
This place is horrible! They let my mother sit in her own urine today because they couldn’t figure out who could change her urostomy bag, not how, but WHO! They also have not been giving her her pain medication on time, even when she asks for it, one day she had to wait 2 1/2 hours for her medication. They also are not cleaning her wounds in a timely matter, shes lucky if it gets cleaned once a day. The only reason im giving them 2 stars is because all of the CNA’s have been wonderful, truly outstanding, the nurses however, not so much.
Citrus Heights Post Acute formaly, Manor Care is the worsted health care facility ever, to send a loved one. They are the most incompetent organization I have experienced!! DO NOT use this facility!!!!!!!
I would highly recommend that DO NOT place your loved ones here. The staff are rude actively display a crude indifference to their residents and visitors. If you currently reside at this facility I would recommend that you to contact your local Onbundsman and the State in regards to your lack of proper care, the denial of your residents rights, the food quality and presentation and overall neglect you have experienced during your placement at this facility
If I could give -5 stars I would!! This facility is so old and nasty! Their beds and furniture are from the 80’s!! I went there after having multi level spine fusion and they treated me like I was able to attend to myself but I couldn’t even get out of the bed because there were no handles to assist me. The food tray was old, dirty and rusty. This place needs to be shut down!! I lasted only 3 hours because the pain was excruciating and I was just left in my bed. I do admit that I feel I was discharged from the hospital too soon. I’m now back at the hospital and getting the right care. DO NOT GO HERE!!
Like all of these facilities they run very lean and are often understaffed. If one person is sick or vaca it is a nightmare until order is restored. The scheduling is phscofrenic at best. The management is way too disconnected from the floor.
Absolutely not worth the 4 star rating on here. No communication from staff, no real rehabilitation for patients, inaccurate records and intake that was not fixed despite multiple attempts. loved one actually came home in worse condition than when they arrived at CHPA unstable and with giant bedsores. Staff were so in hurry to get my loved one out that they miscommunicated the health status, and we had to call an ambulance within an hour of getting them home. When we contacted them the next morning, less than 24 hours after, their response was to dispute the severity of the situation and then say ‘too bad she was discharged’. This is not my first rodeo with post acute and nursing home care, that attitude was not what I have encountered with previous facilities I have dealt with in the past. If you are looking for a place for someone you actually care about don’t pick this one.
I have been blessed with working with Seniors in Assisted Living for 23 years, and was quite alarmed at how this situation was handled by Post Acute Care by Social and Executive Directors. The staffing based upon my experience could be better, but their discharge planning might need drastic improvement. The discharge planning process at Post Acute Care seems to not take the environment to which a patient is being released very seriously. It seems they just dismiss the issue altogether when credible information is provided outlining the unsafe/unhealthy environment in which a patient will be returning to, and they “WILL NOT” inquire or investigate. They in turn choose to dismiss this vital information without any regard to their patients safety and well-being. So for patients that live alone and may not have an in home caregiver, the neighbors and friends are the ones who are aware of any unsafe conditions, if they exist where a patient currently lives. The friends and neighbors are the ones that know the patient intimately. But, THEY DO NOT CARE. IF YOU’RE NOT ON THE HIPA LIST , THEN YOU’RE WORDS CARRY ZERO CREDIBILITY. it just seems quite unprofessional, but more importantly a dereliction of their obligation to provide the best possible care and Stewardship to their patients. I tried to do what would be expected based upon my Assisted Living experience. Based upon my “limited” experience with Post Acute Care, it appears that they either just don’t care or have no interest in listening. It could be a lack of training in Senior Care, but sadly, it’s the upper management that relayed this information. NOTE: i am a mandated reporter by law.
So, this is a little emotional for me, I love Citrus Heights Post Acute. What started for my dad to be here 1 month, will be my dads forever home. The staff are amazing, the day shift nurses, Kim, Heidi, Mandeep, the evening nurses, Alma, Anglica, Nav, Angela. Physical therapist Sophia, registered dietician, Jaymee and the amazing leader of this place Terri. I stop in her office just to say hi, talk about my Tesla, and my about my puppy. She worries about me just as if I was a family member of her own. So my mom needed an operation, me and her took care of my dad, Citrus Heights accommodated. What the surgery led, that my mom had cancer, and ended being my mom, passed away. The staff here have been so supportive, nice, caring, we are a family. I couldn’t imagine placing my dad in a different location. Place your family member here, visit them, treat them like they treated you when you were younger. You will not regret it. If for whatever reason you have a problem. Ask to see Terri. She will help you. You can be rest assured, she will help you and your loved one.
This building is awesome and have taken excellent care of my mom!!
My grandma admitted here after her surgery for physical therapy and continued nursing services. It’s such a bright and clean facility! Staff were always warm and welcoming whenever we came to visit my grandma. They helped her literally get back on her feet. So grateful for the wonderful care she was provided at this facility!
I have been here 3 months now , and the staff treats you like family. The PT&OT department here are very good ! I have learned things here that have improved my life. My case manager and her team are caring, understanding and willing to go the extra mile for anyone, our administration staff, and front office are always open to answering questions, and if you think the questions are not important still ask and they will be more then happy to answer them. Most places the head person is never available for us but ms. Terry is even on vacation! If you are looking for a place for a loved one this is the place I would look at !
My husband has been discharged from Citrus Heights Post Acute for a while now, he is back driving and enjoying his daily normal routine especially doing shopping. My husband and I are truly thankful for this facility! For getting him back on his legs and feet! Great rehab team, special thanks to Claudio! Thank you for the kind treatment and motivation, my husband enjoyed the Tai Chi program. And Thank you to all the nursing team who treated my husband like their family! I will definitely recommend this nursing and rehab place and give them more than 5 star!
My grandfather was at Citrus Heights Post Acute and since has made several statements that he would go back if medically needed. Great therapy staff, they worked hard to get him safe enough to return home both cognitively and physically, also recommended equipment needs to be successful at home. Overall great experience and would recommend to anyone I know needing rehab or long term services.
My experience at Citrus Heights Post Acute Rehab Facility was outstanding. The staff were incredibly supportive and professional always going above and beyond to ensure my comfort and recovery. The physical therapy I received was outstanding, tailored to my needs and delivered with expertise and genuine care. The progress I made was remarkable, and I’m truly grateful for the dedicated team who supported me every step of the way! 5 Stars From Me!
My first night at the facility wasn’t a good experience but eventually when they transferred me to another room, things changed for the better. Rehab program was good and they guided me to my recovery. Staff are friendly, approachable and helpful. I thank them for giving me a comfortable stay. Medical staff are professionals and proficient in their work. They give the best for you. Rooms are cleaned daily. Hygiene kits are provided. TVs are provided for patient entertainment. Free wifi too. The facility allows visitors till 8pm. Good that my family chose this facility. UPDATE Today 3/6 is my last day here. Survived the 3 months stay. Thanks to all who helped me during my stay. I’d like to make a statement of gratitude to the CNAs who pampered me . Shout out to Neha, maria, blanca, raman, nancy, balbir, etc for the care To the nurses sundeep,gurpreet, kristina, tori , mila , kristie etc for administering my antibiotics. To the OT and PT Staff claudio, akiko, kim, bob, ron, holly, miguel, chris etc for the guidance. To the the office staff mel,mary,shelly, etc. Thanks Thank you Citrus Heights post acute
